Goals Of Care Management

We engaged on why the case manager and who the case manager is. Yes, a complex role however very fulfilling. For the case manager to offer quality health care outcomes in the dynamic health care landscape today, it is paramount that they are guided by a case management program which includes some or all of the below:

• Clinical care management (facilitation and coordination of care which is very key for quality outcomes)
• Utilization review and management (includes allocation of resources and ensuring good spread of your benefit)
• Transitional/discharge planning and any arising handoffs!
• Variance management arising from delays in care, omissions or overuse of unnecessary resources.
• Access management and patient flow
• Outcomes evaluation and management
